  2. YOU MUST BRING COPIES OF YOUR RECORDS. The clinic will keep these. Include ONLY those records that support your qualifying diagnosis. Include ONLY ONE OR TWO office visits from a doctor in which your condition was diagnosed or discussed.

  3. There is a $50 deposit when you click the link below. An additional $100 will be due when you check in for your appointment. We accept credit/debit cards or money orders. No checks.

  4. Make sure your valid state-issued ID is current! You can't get a letter of certification without proper ID.

  5. Patients under the age of 18 must have their legal guardian make the appointment and accompany them to their visit.
